I was running a D&D game once and a slip of the tongue caused me to describe an "evil looking omniuos tower". It became a running joke in all our games for years.

The plural of incident is incidents.
The plural of instance is instances.

Incidence refers to the rate of occurrence of something. It probably shouldn't have a plural form, but it can be contrived.

However, I'm increasingly hearing/reading "incidences" when what is apparently meant is incidents/instances.

"Flaunting" rules, used to describe wanton breaches of said rules, where "flouting" would be appropriate.
