You need to put them on your heels. Hence the ancient advice to "cool your heels".
Only on your heels will they cool your liver. If you put them on your liver you are only cooling that bit at the back of your knee that hasn't got a name (excepting for bullshit latin wankery like Popliteal Fossa, but only common and imaginary names have any real value in truly wooful therapies).
